Dr. Carter: Bandages his arm from his dialysis. We find out why he’s back at County. He lost a kidney back when he got stabbed and now he’s contracted some disease in Darfur that attacked his one remaining kidney. Since they don’t have kidneys anywhere in Africa, or any other place besides Chicago for that matter, he’s come back home to get on the donor list. There’s a funny exchange between Carter and Banfield where he asks for one of her kidneys, but he’s just kidding. Carter is a little rusty and out of date with the practicing medicine in a modern E.R.

Neela has a bad day that starts off nicely with breakfast in bed from Dr. Brenner and ends with getting treated like an intern by Dr. Carter and her mentor Dr. Dubenko. She’s so rattled by the terrible day that she decides not to accept the pending job offer from County General.

Dr. Banfield and her husband go to an adoption meeting and her hubby ends up breaking his hand by punching out another potential adoption parent who disrespected Dr. Banfield by telling her he feels sorry for any kid who would be adopted by her.
Dr. Morris treats a couple that drove into Lake Michigan on the advice of their GPS device. They always follow the rules, until Morris starts them on a mattress tag cutting adventure that ends with the husband being run over by a motor scooter. Morris also meets his girlfriend the cop’s new partner from whom he doesn’t get the best of impressions.

Sam treats her mother who is suffering from Alzheimer's and doesn’t recognize her.
Dr. Brenner finally starts developing a character with a real back-story. We’re led to believe though the course of his actions in this episode that perhaps he’s had a pretty traumatic childhood. He goes after foster dad of the little girl whose mother is awaiting a heart transplant. He seems like a nice enough guy but Brenner really lays into him unnecessarily in an attempt to protect the little girl.

Dr. Gates didn’t do much in this episode except shadow Carter and point out all the advancements in modern medicine that Carter wasn’t privy to in his African field hospital.

This episode had some good guest stars. Tony Hale and Judy Greer (Arrested Development) and Amy Madigan. Hale reprised his role as the hero, this time saving a bunch of kids from an apartment fire. He rescued the children’s mother as well, but she died in the O.R. Greer was funny, as the by the book wife who always follows the rules until Morris set her on the path to the dark side. Amy Madigan is always great and this episode was no exception. She’s playing Sam’s estranged mother who doesn’t recognize her own daughter in part due to the stroke and Alzheimer's and also in part because she hasn’t seen her daughter since Sam was sixteen.

It looks like Carter’s life might not be in jeopardy after all. He needs a kidney, but people can live on dialysis for years while they wait to move up the donor list. So that’s a good sign. He’s obviously not going to get a kidney from any of his Family Ties. His grandfather George Plimpton passed away both on the show and in real life. His mom is a dying president on Battlestar Galactica. His brother died of leukemia. Maybe his former heroin addicted cousin can come save the day since Carter saved his life after his overdose and he hasn’t been doing much since Raising the Bar got canceled.
