Truth is the theme of the episode. Somehow, all of the characters grow to a truth, admit a truth or become bluntly truthful. Newsflash! Really Old Guy, aka Charlie, wakes up. He is the center for the truth, explaining the actions of the rest of the characters. He tells Izzy, who is taking care of him, that he wants to die, that there was nothing worth living for. He also tells her that he doesn't think that George will leave Callie. Though, the last few minutes of the episode might preview that Callie will leave George instead.

Bailey just keeps continuing to grow. She puts Alex in line with teaching his intern and Meredith in line with her sister. Alex realizes that he should be truthful in all aspects - and the nice Alex disappears as he outs George's repeat of internship in an elevator of interns. Meredith confronts her sister after arguing with her. Bailey comes to a compromise with Callie, telling Callie that they will work as a team and Bailey will accept her role as Number 2.

For once, I was disappointed with Cristina's portrayal. Sure, so we do hear a confession about how its becoming this reality that Burke is really gone. However, I think it is too early for her to be manipulating her sadness to fake her way into getting surgeries. Next episode, maybe, but this episode? It's just too close to season premiere still.

Speaking of Burke, Grey's lost 18% of the viewers of last year for the season premiere, according to Entertainment Weekly. Is the exit of Isaiah Washington really the root of the loss? Or is it that people are just becoming bored with watching Grey's? I wonder ...

Also, I am unsure of the whole pushing Alex with Lexie. Alex would just become another McDreamy without the nickname. More on that later if it develops into something.

And so the episode concludes with Really Old Guy dying after he checks himself out of the hospital. The last few moments are dedicated to the largest explosion of truth - George's confession of sleeping with Izzy to Callie - and viewers are told to stay tuned. Right now, it's the largest storyline to stay tuned into.
