Remember two weeks ago when the whole point of the episode, in regards to JD, was that he needed to grow up? Yeah, I guess those wacky writers (support the WGA!) felt they needed to clarify that a bit more. Thus, we have this week’s episode.

The episode opens with one of the most disturbing daydreams ever conceived in the 7-year history of the show. Let’s just say it involves Sammy (JD’s son), all the males in the hospital having large lactating breasts, a milk fight, and Turk’s prosthetic nipple. Did they really just show that? Whoa.

Back in reality, JD and Turk have some fun while spending time with their children: goofing off like in the good old days, except with babies in tow. They make plans for their day-off to be the “World's Most Giant Black Doctor” complete with giant stethoscope and clipboard for taking a somewhat Heathers-esque lunchtime poll. JD suddenly remembers that he’s supposed to grow up and chastises Turk for not taking his own advice given last episode. Hurt, Turk feels that growing up doesn’t mean that you have to lose this kid inside you. Carla agrees and after a stern talking to, JD comes around as well. Hello, “World's Most Giant Black Doctor.”

On the other side of the hospital, it is Dr. Kelso’s birthday and Elliot, Ted (yay!), and The Janitor are determined to find out how old he really is (Kelso has been saying he is 58 for years now). The suggestion to cut him open and count the rings is my favorite. They settle on sneaking a peek at his personnel file and discover that he is 65. A birthday party (decorated with “65” everywhere) later, a board member approaches Dr. Kelso. This doesn’t look good. Apparently, when a Chief of Medicine turns 65, he/she is phased out. Yikes.

On the Perry side of things, Dr. Cox feels it’s time for Jack to grow up and refuses to do “the funny voice.” He is also treating a leukemia patient (Josh) whose parents refuse to tell that he has the disease. Dr. Cox thinks this is wrong, that the kid has a right to know, and that he is old enough to understand what is happening. So what does he do? He goes against the parents (and hospital policy) and tells Josh what’s up (can you say Dr. Ross from ER?). Stealthily avoiding a lawsuit, it is Carla who is really pissed at Perry. Why? He took away Josh’s childhood. Later that night, Dr. Cox does the funny voice for Jack.

In a small little side story, Dr. Hooch goes crazy. Fun times. To recap, the “growing pains” in the episode are as follows: JD trying to grow up, Dr. Kelso trying to NOT grow up and straight into retirement, Dr. Cox trying to make his own son and a young leukemia patient grow up too fast. Got it? Good.

Next on Scrubs: Kelso signs Sacred Heart up for RateYourDoctor.org, a website where patients evaluate their doctors. That sounds like fun.
